Dear Patients of Dr. Sarah Hodges,

We wish to inform you that Dr. Sarah Hodges will be concluding her practice with Pemberton Primary Care Society as of March 13, 2026, as she and her family will be relocating to the UK to be closer to family.

Dr. Hodges has greatly valued the relationships she has built with her patients and the community, and her contribution to the practice will be sincerely missed.

All patients currently under Dr. Hodges’ care will continue to be patients of Pemberton Primary Care Society and will be offered ongoing care with one of our other practitioners who are currently accepting patients. Our team remains committed to ensuring a smooth transition of care.
